Discharge Follow-up Phone Calls
To help us improve the way we care for our patients and to help ensure you understand the doctor's instructions, we make brief random follow-up calls to some of our patients.
Our physicians hold privileges at Kosciusko Community Hospital in Warsaw, Indiana.
To assist you in establishing a primary care provider, please call our new patient line at (844) 564-2778(844) 564-2778.
After Hours Needs
If you believe your problem is life threatening, go directly to the emergency department or dial 911.
After office hours, you may reach the physician on call for urgent care needs by contacting the physician answering service at (888) 442-1044 (888) 442-1044 . The operator will ask the reason for your call. A physician will return your telephone call.
Reserve your calls for medical care needs that cannot wait until normal office hours. Do not call the answering service for medication refills, test results or to schedule an appointment. Pain medications will not be refilled during the weekend.
On weekends, the family practice on call physician may be from any of the following offices:

If you need to cancel your appointment, please give us 24 hours notice when possible. Three or more missed appointments without contacting the office to cancel beforehand could result in your being released from the practice.
Work/School Slips & Forms
Patients must be seen by the physician to obtain a work or school slip to document disability or absence. Excuses will not be given for days of work or school missed prior to your office visit.
In general, an appointment will need to be scheduled for the completion of forms such as FMLA, etc.
Medication Refills & Requests
For medication refills, please bring all bottles including over-the-counter medications and any vitamin/herbal supplements to each visit.
We will prescribe enough medication to last until your next appointment. If you are on your last refill, please call to schedule an appointment.
To refill certain medications, you may be asked to schedule an appointment prior to refilling. A return visit is essential in determining the effectiveness of your medications and health care plan.
Please call 48 hours in advance of your refill needs.
Pain medications will not be refilled without an office visit.
